OSTEOBIZ BALANCE

Your income shouldn’t feel like a seesaw

One month the diary is full and you wonder why you ever worried. The next it’s patchy, the gaps appear by Wednesday, and you find yourself checking the bookings three times a day hoping something lands.

Same clinic. Same skill. Same reputation. Completely different month.

Most owners put it down to luck, or the time of year, or something they must have stopped doing. It’s usually none of those.

Your planning session is the first thing that happens when you join

Over 20 clinic owners across the UK and beyond are already inside Balance.

Building steadier, more sustainable clinics without the noise, the pressure or the guesswork

Meet Bob

Bob is an associate. Competent, kind, genuinely good with patients. His diary is half empty and nobody can work out why.

Here’s why. Bob dreads the last two minutes of an appointment.

He knows the patient needs a course of care. He also knows what it costs, and the thought of asking a stranger to hand over more money makes him feel like he’s reaching into their wallet. So he says “see how you go” and lets them decide.

They go away. Some come back. Most drift, wonder whether they should have gone to the GP for a physio referral instead, and you never hear from them again.

Bob isn’t lazy and he isn’t underskilled. He simply has no words for that moment that don’t feel pushy, or salesy, or give him the ick.

Almost every clinic has a Bob. Some have three. And the cost of it doesn’t show up anywhere on a spreadsheet, which is exactly why it goes on for years.

It isn’t a marketing problem

When the diary looks thin, the instinct is to go and find more patients. More posts, more ads, more effort at the top.

But if the clinic is leaking patients the way Bob’s is, more marketing just pours water into a bucket with holes in it. You work harder, you spend more, and the diary looks much the same.

You already know that structure governs function. A clinic’s results come from the way it runs behind the scenes, not from how loudly it markets.

Get the engine room right and things start to compound instead of resetting every month. That’s the whole idea behind Osteobiz Balance.

And if you’re already full?

Then you’re the person who gets the most out of this, which surprises people.

Because busy is when the real questions start.

What you charge, and whether it has kept pace with your experience. What your associates actually do when you aren’t in the building. Whether any of it would survive you taking a month off. Where the money goes, and why the bank balance never quite reflects how hard the year felt.

A full diary isn’t the finish line. It’s the point at which the interesting problems arrive, and most owners face them on their own.

What you get, week to week

Weekly drop-in sessions with me and the team. Bring the thing that’s been sitting on your shoulders and leave with a straight answer, often with an extra idea for revenue you hadn’t spotted. Come every week, come just to learn from others or come when you need to.

The members’ WhatsApp group. For the question you’d feel daft asking anyone else, answered the same day, often by another principal who solved it last year.

Two in-person days a year, the last Thursday of May and the last Thursday of November. At my place, and on Zoom if you can’t travel. A full day to take a deep dive into your business and think about the bigger picture instead of reacting to whatever the week threw at you.

The group runs from sole practitioners working out of a room at home to large multidisciplinary clinics with full teams. That mix is the point. The questions overlap far more than you’d expect, and the advice flows in every direction.

The masterclass cycle

Six live masterclasses run through the year, every other month, each one drawn from my book The Clinic Engine Room.

29 October 2026 – Keeping your team fully booked

Bob. The words that make rebooking feel like care rather than selling

10 December 2026 – What’s really stopping you putting your fees up

Why your fees are where they are, and how to change them without losing patients

25 February 2027 – Flying blind: The four numbers that run your clinic

Knowing your position instead of guessing at it

29 April 2027 – How patients find you now

What search and AI actually say about your clinic, and how you appear across the whole of the internet

10 June 2027 – The patient list that looks after itself

Building the list so it stops being a spreadsheet nobody touches

26 August 2027 – The extra revenue hiding in plain sight

The money already inside the clinic that nobody has gone looking for

Then it comes round again.

That matters more than it sounds. This is a cycle, so there is always something next.

Every masterclass is live, with your questions in it, and every one points you to the chapter that holds the full version.

October is free for members’ whole teams

Not one seat. All of them.

Bring every associate, every practitioner, the receptionist who books people in. Sit them down together and let them learn it at the same time, which is the only way a change like this ever sticks in a clinic.

Picture the version of your team that comes out the other side. Every one of them ending an appointment with confidence, explaining a course of care so the patient feels properly looked after rather than sold to, and nobody leaving your clinic wondering whether they ought to ring the GP about a physio referral instead.

That’s what your Bob is missing. It takes less than an hour to give him the confident words he needs.

Three people who sit in these sessions

Ruth works from a clinic room at home. She came in with a diary that never quite filled and fees she hadn’t looked at in years. She sorted the engine room first, then put her fees where they should have been. She is now fully booked.

Mike started as a private client and moved into Balance when the one-to-one work finished. He uses the sessions for the things that keep clinic owners up at night: who to hire, how to manage a team he barely sees, what to do when recruiting without making a costly mistake.

Sarah runs a large multidisciplinary clinic and doesn’t need anyone to teach her the basics. She uses Balance as a sounding board, because at her level the decisions are big and there is nobody inside the business she can think them through with.

Three very different clinics. The same room, every week.

Where this gets you

Picture yourself a year from now.

The diary doesn’t lurch. Your associates rebook confidently, without being chased, because they finally have the words. You know your numbers instead of guessing at them, and the bank balance has started to look like the effort you put in.

The clinic holds together whether you’re in it or not.

And you go away without watching your phone.

Your Investment

Six months in full – £1,125 including VAT.
One month on us

Twelve months in full – £2,250 including VAT.
Two months on us, and all six masterclasses from the top

There is no joining fee.

Everyone who pays in full gets half an hour with me, one to one, every six months. Normally that’s only for my private clients.

On the fee. One or two patients coming back for the rest of their course of care, instead of drifting off after the first appointment, easily covers the month. A well-engineered engine room manages that gently and automatically in the background, which means it keeps covering it long after you’ve stopped thinking about it.

How often are the sessions?

Drop-ins run weekly. Masterclasses run every other month, six a year. In-person days run twice a year, the last Thursday of May and the last Thursday of November. That’s the whole calendar.

Do I have to come every week?

No. Come when you have something to bring. The drop-ins are there whenever you need them, not an obligation sitting in your diary. But many do drop by with a sandwich, just to soak up our discussions.

I'm a sole practitioner. Is this too big for me?

No. Some of the people who get the most from it work alone from a room at home. The fees conversation, the numbers, the patient list, the way you end an appointment: none of that needs a team.

I run a large clinic with a full team. Is this too basic for me?

No. At that size the value is usually having somewhere to think out loud with people who understand the decisions, plus getting your whole team trained in one go.

What happens after three months?

It simply rolls on monthly. Most members stay for years, because the cycle keeps turning and there is always something next.

What if I miss a masterclass?

They’re recorded, and members keep access. Come live if you can, because the live questions are half the value.

Can my team come to the others, not just October?

October is the one that’s open to your whole team. The rest are for you as the owner.

Come and join us

You already know what needs doing. The hard part is doing it on your own, between patients, at the end of a long week.

That’s the bit Balance takes off you.

Thirteen years and more than 1,100 clinics have taught me that the fastest thing to change in any clinic is what happens in the last two minutes of an appointment. That’s October, and your whole team is invited.

My Clients have A Lot To Say

“I had a clinic group webinar with Gilly today, and she was absolutely fantastic. She was engaging, informative and inspiring. I feel like I’ve got some great ways to improve my online presence, and I look forward to putting it into practice. She’s given me the get up and go and the confidence that I severely lacked regarding online posting!”

- EMILY

“I almost wish I’d never met Gilly!!! I am so busy I can hardly keep up with my patients, I only have to talk to her and more seem to come out of the woodwork, she’s a whizz at what she does. Thank you so much for all the fantastic advice, help and support”
